The BCBSM Coordinated Care app is designed exclusively for Blue Cross Blue Shield of Michigan members who have received an invitation to join the app as part of the Blue Cross Coordinated Care program. In order to sign up, you will need your Enrollee ID and an access code.

Once you're a member of this program, the BCBSM Coordinated Care app provides a range of valuable features to help you take control of your health and well-being. Some of its key features include:

 <li>Monitoring of your health progress and overall well-being</li>

Blue Cross Coordinated Care is an all-in-one, personalized support program that prioritizes the well-being of its members. The best part? There are no additional costs associated with this program.

If you have received an invitation to join the program, you will have the opportunity to work closely with a registered nurse who is part of your care management team. The BCBSM Coordinated Care app makes it easy for you to virtually connect with your nurse and access a wide range of helpful resources.

Please note that BCBSM Coordinated Care is compatible with Apple Health for certain care programs that involve step count tracking.

Remember to consult with a doctor before making any medical decisions. This app is designed for educational purposes and as a means of support between provider visits. If you are enrolled in an eligible care program, you may connect a glucometer to share readings with your care management team. However, it is essential to continue consulting with your doctor before making any changes to your diet, exercise routine, or treatment plan.
